You will be requesting a letter from your doctor, which MUST STATE
that you have a current PTS diagnosis,
that you are currently in treatment for PTS,
that a Service Dog is medically necessary, and
that you are therapeutic enough to participate in a 52-week dog training program.

After review of your questionnaire, AVSDA will send a confirmation email to you, with instructions regarding scheduling an interview with our Program and Training Directors. Following this interview, you will be instructed to complete the full application process.
