Letter of Referral from a Mental Healthcare Provider

You will be requesting a letter from your doctor, which MUST STATE that you have a current PTS diagnosis, that you are currently in treatment for PTS, that a Service Dog is medically necessary, and that you are therapeutic enough to participate in a 52-week dog training program. You don't need the letter yet, but it will be a requirement.

DD214 with Honorable Discharge

This is solely for proof of service.

Completed Dog Application

There are links to two different types of dog applications on the Admission and Registration page; you will only complete one, based on whether you want to train a dog you already have, or train a dog you are seeking through AVSDA. Again, we don't need you to submit your dog application yet; after completing the Veteran Questionnaire, below, we will instruct you on next steps. We do not admit dogs that do not meet our dog admission criteria.

After review of your questionnaire, AVSDA will send a confirmation email to you, with instructions regarding scheduling an online/ Zoom interview with our Program and Training Directors. Following this interview, you will be instructed to complete the full application process.
