How to Apply to AVSDA

We offer two options for registration...
Upon admission, AVSDA will provide you with fully-funded access to our 52 week service dog program. Depending upon where you reside, your wellness, and other factors, you may enroll as either a 100% in-person student, or as a hybrid (online and in-person) student.
Students enroll as applicants seeking a dog through AVSDA. We do not place AVSDA dogs to applicants residing more than 50 miles from Manteno, Illinois; however we can guide you in your selection process of a suitable prospective service dog
As an enrolled student, you will receive some training equipment you'll need for participation in our program; any extra items you desire are not provided but you can purchase them yourself. We hold a strict policy about approved training equipment; use of unapproved equipment is grounds for removal from the program.
Please note, we do not adopt dogs to program participants until they meet all criteria. Most students go through an adoption ceremony immediately following their graduation.
Our registration process requires you to follow a series of steps to complete all of the application materials. This can seem daunting, and there are quite a few forms to fill out and submit; but don't be discouraged! We are here to walk you through the process and guide you every step of the way. Take your time and go at your own pace. If you have a caregiver or family member who might be willing and able to help, consider asking them to help you complete the registration process.
